CPAP reimbursement Belgium - RIZIV/INAMI scheme 2025

CPAP reimbursement in Belgium - understanding the RIZIV/INAMI convention

Have you been diagnosed with sleep apnea and are wondering how to get a CPAP device in Belgium? You may have heard about "RIZIV reimbursement" or "INAMI reimbursement" and want to understand what it really means. This guide explains the Belgian system in full: how it works, who qualifies, what it costs, and what your options are if you don't qualify.

Note: RIZIV (Dutch) and INAMI (French) refer to the same Belgian institution: the National Institute for Health and Disability Insurance. The CPAP convention is identical for all Belgian residents, regardless of language.

What the Belgian CPAP convention actually is

The Belgian CPAP convention is often described as a "reimbursement", but this is misleading. It is not a purchase reimbursement. The system works differently:

What the convention is NOT

  • You do not buy a CPAP device
  • You do not choose the model
  • The device never belongs to you
  • Your health fund does not reimburse a purchase

What the convention IS

  • An accredited sleep centre provides the device on loan
  • Your health fund pays the centre directly
  • You pay only a small patient contribution
  • Device + mask + follow-up: everything included

The accredited sleep centre takes care of everything: the device, mask, tubing, filters, heated humidifier if needed, medical follow-up, maintenance and repairs. Your health fund (mutualité) pays a daily rate directly to the centre. You pay only a patient co-payment of €0.25/day, i.e. €7.50/month - or nothing at all if you have the BIM/OMNIO/increased intervention status.

Eligibility conditions for the RIZIV/INAMI convention

To benefit from the Belgian CPAP convention, you must meet three cumulative conditions:

Condition Details
1. Diagnosis by polysomnography (PSG) Full overnight study at an accredited sleep centre. Required result: OAHI ≥ 15/hour (Obstructive Apnea-Hypopnea Index). This threshold has been in force since the 2017 amendment 5.
2. Proven CPAP efficacy A second examination (home or in-centre titration) must confirm that CPAP improves your breathing.
3. No more appropriate treatment available The sleep centre physician must verify that no alternative treatment is better suited to your case.
The 4-hour rule: Once on the convention, you must use your CPAP for an average of at least 4 hours per night to maintain coverage. This is an average over the entire reference period - not a nightly requirement. Falling below this average triggers a 3-month "joker" agreement. If compliance is still insufficient after the joker period, the convention is suspended for 1 year.

CPAP convention costs

Status Per day Per month Per year
BIM / OMNIO / Increased intervention €0.00 €0.00 €0.00
Standard beneficiary €0.25 €7.50 €91.25

This amount covers everything: the CPAP or Auto-CPAP device, mask, headgear, tubing, filters, humidifier if required, maintenance, repairs, replacement in case of breakdown, and complete medical follow-up.

Step-by-step journey through the convention

  1. Your GP suspects sleep apnea (snoring, chronic fatigue, daytime sleepiness, observed breathing pauses) and refers you to an accredited sleep centre.
  2. Initial consultation at the centre: full medical history, clinical examination, risk factor assessment.
  3. Polysomnography (PSG) in the sleep laboratory: one full night with sensors measuring brain waves, breathing, oxygen levels, heart rate. If OAHI ≥ 15/hour → diagnosis confirmed.
  4. CPAP titration: efficacy test, either at home with a loaned Auto-CPAP or during a second night at the centre.
  5. Application to your health fund: the centre submits the complete file. Response time: a few days to two weeks.
  6. 6-month initial agreement: first coverage period. Renewals of 12 months if the 4h/night average is met.
  7. Equipment handover and training: the centre gives you the device, fitted mask and all accessories, plus full training on use and maintenance.
  8. Regular follow-up: the centre monitors your compliance, treatment efficacy and equipment condition.
Total timeframe: Allow 4 to 8 weeks from your first consultation to CPAP delivery at home, depending on appointment availability and your health fund's response.

What if you don't qualify for the convention?

The RIZIV/INAMI convention does not apply to all patients. You may wish to purchase your own CPAP device in several situations:

  • OAHI < 15/hour: you don't reach the required threshold, but your doctor still recommends CPAP therapy.
  • Immediate start: you don't want to wait 4 to 8 weeks for the conventional pathway.
  • Freedom of choice: you want to select your own device - the latest connected model, the mask that suits you, the accessories you prefer.
  • Device ownership: you want to own your CPAP, not borrow it. Take it anywhere, including on holiday.
  • Second device: you're on the convention for your main CPAP but want a compact device for travel.
  • Direct data access: modern connected devices give you full access to your therapy statistics without going through the centre.

Frequently asked questions about the RIZIV/INAMI CPAP convention

Three cumulative conditions are required: a diagnosis by polysomnography (PSG) at an accredited sleep centre showing an OAHI ≥ 15/hour, proof that CPAP effectively improves your breathing (titration), and the absence of a more appropriate alternative treatment. If any of these criteria is missing, you cannot enter the convention. This is the official Belgian threshold since amendment 5 of 2017.

The OAHI (Obstructive Apnea-Hypopnea Index) must be at or above 15 events per hour, measured by full polysomnography. It differs from the international AHI by counting only obstructive apnoeas and hypopnoeas, not central ones. To understand this important distinction, see our page AHI: apnea-hypopnea index.

The patient co-payment is €0.25/day, i.e. roughly €91.25/year or €456 over 5 years. Holders of the BIM/OMNIO/Increased Intervention status pay nothing. This covers the device, mask, tubing, filters, humidifier, follow-up, repairs and replacements. Note: this is not a purchase. The device never becomes your property.

Yes, everything is included in the package: CPAP or Auto-CPAP device, mask (and regular replacements), headgear, tubing, filters, humidifier if needed, maintenance, repairs and medical follow-up. The accredited sleep centre manages the entire kit. The patient pays no extra beyond the daily co-payment.

You do not qualify for the RIZIV convention, but your doctor may still recommend CPAP if you have disabling symptoms (chronic fatigue, sleepiness, hypertension). You can then buy your own device directly, for example from VivaRespire. You choose the model, start immediately, and own your equipment for life. To understand your AHI: AHI page.

Yes, polysomnography at an accredited sleep centre is reimbursed by INAMI as part of the diagnosis, regardless of CPAP convention eligibility. You usually pay the standard specialist consultation co-payment. To find an accredited centre near you, see our directory accredited sleep centres in Belgium.

The device goes back to the sleep centre: it has never belonged to you. The 4-hour-per-night average rule must be respected to stay on the convention. If breached, you get a 3-month "joker" period. If compliance remains insufficient, the convention is suspended for 1 year. If you then wish to own your own equipment, buying at VivaRespire is the natural alternative.
